INSTRUCTIONS :

This application is 4 pages long with easy to use typeover forms formatted in Microsoft Word. Shaded boxes will expand to fit your text and you can tab to the next field. All pages must be typed on a computer and emailed or saved on disc and mailed to: Conference Coordinator, PCA Louisiana, 733 E. Airport Ave., Suite 101, Baton Rouge, LA 70806. Hard copies alone will not be accepted. Your ORIGINAL signed Step 4 (Vested Interest form) must be mailed.

Step 1 : Fill out document one on computer: Workshop proposal application.
Step 2 : Fill out document two on computer: Biographical data form (for each presenter).
Step 3 : Fill out document three on computer: ED1 Educational Design form.
Step 4 : Fill out document four on computer: Vested Interest form. Sign and mail.
Step 5 : Email to Abrunson@pcal.org or save on disc and mail to:
